Below you will find the forms you need to fill out for any artist to pierce or tattoo either you or your child.

In order to get a piercing or tattoo under the age of 18 you must be accompanied by a parent or guardian- At which point they must fill out the necessary form in its entirety.



In order to be tattooed, the minor must be above the age of 16.

GENERAL CONSENT FORMS

In order for our artists to pierce or tattoo any patron they must fill out the necessary consent form in its entirety.
